Notice to Patients This practice serves all patients regardless of ability to pay. Discounts for essential services are offered depending upon family size and income. You may apply for a discount by asking your therapist.
NOTICE This practice has adopted the following policies for charges for behavioral health care services. We will charge persons receiving health services at the usual and customary rate prevailing in this area. Health services will be provided at no charge, or at a reduced charge, to persons unable to pay for services. In addition, persons will be charged for services to the extent that payment will be made by a third party authorized or under legal obligation to pay the charges. We will not discriminate against any person receiving health services because of their inability to pay for services, or because payment for the health services will be made under Part A or B of Title XVIII (“Medicare”) or Title XIX (“Medicaid”) of the Social Security Act. We will accept assignment under the Social Security Act for all services for which payment may be made under Part B of Title XVIII (“Medicare”) of the Act. We have an agreement with the State agency which administers the State plan for medical assistance under Title XIX (“Medicaid”) of the Social Security Act to provide services to persons entitled to medical assistance under the plan.
How long will treatment last? Treatment begins with a comprehensive assessment. Information obtained from the assessment will be used to develop a treatment plan. Individual treatment length is determined by an individual’s progress with his/her treatment goals. Actual length of treatment may vary.

If you have other insurance, not listed, please call to verify coverage. It is always best to contact your insurance provider prior to your first appointment to verify coverage.
How are therapists selected for certain clients? Individuals seeking treatment may choose any available therapist. Every effort is made to accommodate an individual’s preference for a therapist, whether male or female. Therapists may have a particular area of experience or interest that will be best suited to meet client’s needs. At BCC we try to meet the individual’s needs with an appropriately skilled therapist.
Court ordered to receive treatment? As a licensed Outpatient Drug and Alcohol facility, BCC can provide court ordered treatment to individuals with a drug or alcohol related offense. In the event that an individual is required to complete a Mental Health Assessment, BCC can provide that service.
Will everything be kept confidential? As a medical provider, we are required to adhere to all State and Federal Regulations regarding a client’s confidentiality. If you desire to have information shared, we will require you to sign a consent to release information. This will allow us to share the information specified on the form, and in accordance with regulations to whoever is requested.
